What to expect the first visit?

An initial evaluation includes review of your medical history, a functional/ movement examination, ***an internal pelvic floor examination performed intravaginally or intrarectally to see how pelvic floor muscles are functioning***, have a conversation about your goals for physical therapy, answer any questions you have, and develop a treatment plan that is evidence-based, holistic, and individualized to address your goals.

***Internal evaluation and assessment only performed if appropriate based on the individual***

Where are you located?

Office located within Simpson Family Wellness located at 4922 W Seneca Turnpike, Syracuse, NY 13215 **Select days and times available**

**Mobile visits are available as needed**

Do you accept insurance?

Empowered Physical Therapy PLLC is an out-of-network provider which means the length of your visit is NOT limited by insurance. I am able to provide one-on-one evaluation and treatment sessions. A detailed receipt will be provided to you upon request for you to self-submit to your insurance. Please review your out of network benefits prior to your appointment if you have questions regarding reimbursement.

Do I need a physician's referral?

New York is a direct access state which means that you are legally permitted to see a Physical Therapist for evaluation and treatment without a physician's referral (prescription). Treatment can be rendered for up to 10 visits or 30 calendar days, whichever comes first. Please check with your individual insurance provider to see if they require a referral for reimbursement.

Can I use my HSA/FSA?

Yes! Physical therapy does qualify as an approved expense, even under the cash-based model. Please review the rules and regulations of your HSA or FSA before your appointment.
